Introduction

It is particularly important for patients to know that the upper eyelid must be viewed and analysed as an aesthetic and functional unit overall.

This is why the preliminary evaluation is particularly relevant:

  • Analytical evaluation of the eyebrows.
  • Examination of the muscle that lifts the eyelid

Many patients who come to our practice for upper eyelid plastic surgery also have problems with the eyebrows and occasionally also with the eyelid lifter muscle.

To adequately recognize eyebrow lifting without possible problems of the eyelids or eyelid lifter muscle can unfortunately end in a surgical, medical, psychological and costly adventure.

Occasionally it is indicated that an eyelid lift with or without correction of eyelash lift weakness can achieve an excellent aesthetic and functional result. However, it is also possible that the cause of your discomfort is a lowering of the eyebrows.

Men in particular usually have heavier eyebrows than women. However, often a combination of age-related symptoms of the eyelids, eyebrows and other structures is also present for a multifactorial genesis of the changes in these patients.

Methods of brow lift

There are very different methods of eyebrow lifting as described above. A very individual surgical treatment plan must always be worked out, depending on the degree and severity of the signs of aging and pathological changes.

The methods used are also very different and are discussed in detail in advance. Factors influencing the method are:

Where are the cuts?

With the eyebrow lift, the incisions are in different places. There are 4 different incisions, which are also carried out differently depending on the surgical technique.

1. just above the eyebrows

A well set incision directly at the upper eyebrow border, so that the incisions are almost invisible.

2. mid forehead

Depending on the horizontal forehead wrinkles, these wrinkles are available so that cuts can be integrated into the wrinkles almost invisibly.

3. pretrichial

Wave-shaped cuts which are placed along the hairline above the forehead in a certain way so that these scars are not visible.

4. endoscopic

Several small vertical incisions which lie beyond the hairline between the hairs of the hairy scalp.

Which incisions and which surgical techniques are advisable will be discussed with you in great detail and on a large scale during the consultations with Dr. Scholz.

Here also there are different procedures and surgical planning, which can be very individual and different in each particular case.

Are there scars?

Since the surgical incisions are made in very discreet areas around the eyebrows or on or beyond the hairline, scarring is minimal and usually almost invisible.

The scars fade with time until only wafer-thin lines are visible that are almost invisible to the naked eye.

People who are prone to excessive scarring (keloids) should discuss this with us. There are treatment options that can be used prophylactically before an operation.

In addition, special attention must be paid to these patients during the post-operative period in order to carry out appropriate scar care and treatment, which can prevent the development of wide scars or excessive scars.

Eyebrow lift or Eyelid lift

It is very important to understand that eyelid plastic surgery without analyzing and, if necessary, treating the eyebrows can result in a major avoidable problem for the patient.

For this reason, it is not uncommon for patients to visit me for an eyelid plastic surgery, but in reality they need a correction of the eyebrows (e.g. in combination with an eyelid lift).

Surgical Procedure

Arrival at the clinic on the day of the operation takes place approximately 1 to 2 hours before the actual operation. If necessary, examinations will be carried out here.

As a rule, the operation lasts between one and three hours, is performed under local anesthesia with or without twilight sleep, or under general anesthesia and does not require an overnight stay in the clinic.

The operation begins with small incisions directly above the eyebrow, at medium height of the forehead, at the height of the hairline or with minimally invasive endoscopic treatment beyond the hairline.

Then the excess skin is removed, the underlying muscle is partly narrowed, the soft tissues are repositioned, possible additional procedures are performed (e.g. eyelid lifting) and finally the incisions are closed with dissolvable sutures.

This means that the sutures, most of which are covered by plasters, do not have to be removed.

You will then remain in the clinic until you have fully recovered from the procedure. You will be discharged from the clinic after consultation with the surgeon and the anaesthetist. If desired, an inpatient stay can be arranged, but discharge on the day of the operation is usual.

Costs

Prices and costs for an eyebrow lift vary according to complexity. Patients often confuse eyelid correction with eyebrow lifting or need both.

Healing process

Cooling the eyebrows and forehead is a very helpful thing and supports the early phase of wound healing (within the first 24 hours). However, care should be taken that very cold cooling aggregates or ice cubes do not come into direct contact with the skin in order not to injure the tissue.

A mild to moderate amount of swelling can be expected in the area of the eyebrows and forehead on the first day.

Follow-up checks are carried out at regular intervals after the operation. Wound healing is a process that lasts at least 1 to 1 1/2 years. For this reason, long-term follow-up checks are a matter of course for our patients, make medical sense and in our eyes are an absolute service and a great pleasure for us to analyse the long-term results together with you.

Side effects

Pain

Pain perception is a completely indivdual quantity. While sensitive people complain about pain, more insensitive people easily put the pain away. However, as this is a surgical operation, postoperative pain and wound pain must be controlled with medication.

Today, there are a variety of very intelligent pain treatments that allow a quick return to normal daily activities.

Swelling

The swellings usually decrease from the 4th day after the OP. After approx. 2 weeks 80% of the swelling has disappeared.

The remaining 20% of the swelling will continue for another 4 weeks until it has completely disappeared.

Scars

We address this topic in more detail above. In general, scars will hardly be visible. Persons with a tendency to scarring (keloids) need special treatment and are an exception.

Conclusion

The eyebrow lift requires a feeling and experience of the plastic surgeon, as well as profound knowledge of the surrounding structures. In addition, the doctor must understand the patient's face aesthetically and have surgical techniques to achieve visually harmonious symmetries.
